We hope you find the information on this page to be helpful in planning for your time at The Theater Barn. The first section contains important information about what to expect concerning communication from us. The other sections have information on transportation to New Lebanon, typical rehearsal schedules, and businesses in and around New Lebanon.
Paperwork & Exchanging Information:
CONTRACT: If you are on this page, you should have already e-signed your contract. You will receive a PDF of your signed contract by email for your records. The e-sign process is managed by SignNow, so be sure you check your email and spam folder if you do not see it.
W-9: You will receive a SignNow version of the IRS W-9 Request for Taxpayer Identification Number. You will need to complete this so we are able to issue you a paycheck. It is important that the address on this form is an address that you can receive mail at when tax information is sent in January of next year. If your address changes, please notify us. Once you complete an electronic W-9 with us, in most cases you will not need to complete a new W-9 if you work for us in the future. The information on this form is relatively simple to complete, but we are required to send you the full form with instructions from the IRS. If you have questions on how to complete the form, please consult the instructions on pages 2-6. While we cannot help you with how to answer the questions, we can help you with the electronic completion of the form if you have questions. COSTUME MEASUREMENT SHEETS: Actors and some on-stage musicians will receive a measurements form to complete to be shared with the costume department. Most fields on this form are optional, so if you don't think these apply, you can leave the question blank.
